Actually, he may have a condition known as gynecomastia. Causes the glandular tissue of the breasts to develop in males. It can be caused by LOTS of things (drugs, liver problems, etc.). As a physician, I can assure you that gynecomastia is NOT the case here.
It results in a relatively small amount of tissue, usually not sufficient to fill out an A-cup bra.
Notably, there are an increasing number of older males opting for plastic surgery and unfortunately, there are unethical physicians who are warped enough to perform the procedure on mentally ill individuals as long as they have the money.

I hope the person who is saying it is his grandfather is telling the truth.
